Instructions for completion of study survey

to take you to our survey site.
When you log in you will create a profile with basic information:
Name, Email, Organisation, Telephone number. It will ask you what kind of organisation you are. (A&E, Primary Care Provider or PCT commissioner).
Please select the correct one.
You will have to choose a log in name and password. Please take a note of these.
You will then be sent an email which asks you to click a link to validate your use of the site. The link will take you back into the site and you will have to log in.
Then click on Start Survey.
The next screen will ask you to add a hospital site. Put the name of the site where asked and the survey questions will be revealed.
This is to allow organisations with more than one site to complete the survey for multiple sites. You can add as many as you have. Then click edit and the survey will be presented.
This is divided into sections. When you complete a section and you move to the next you save it automatically. You can leave the site if you want and come back later.
We recommend that you use the 'print' function before and after entering data. The initial version will provide you with a .pdf and/or hard copy of the questionnaire so that you can get the answers ready. The final version makes it easy to check the answers and allows you to keep a record of your response.
Only when you have completed all sections can you submit the answers. Once submitted you cannot edit. You can go back and edit prior to this final step.
